I vaguely recall hearing that applications for dual members have to be
faxed. Is that still true, or is it now possible to submit them online?

According to the club management page

Reinstated, transfer and dual members cannot be submitted online. Please
fax or mail them with payment to World Headquarters.

I'm the Treasurer of an advanced club where most of the members are dual, so
I have to deal with this all the time. Applications for dual members cannot
be submitted online. You need to fill out the paper form and either mail or
fax it. Something I tried recently was to scan the form into a PDF document
and email it to WHQ, and they did accept this.

So you sent someones credit card information in an uncrypted form via
email?

I think it'd be acceptable to scan the application and then call in with
the credit card information but not put it in an email unless you're
going to encrypt it.
